Hi, I'm Dr. Leifer. I am both a dermatologist and a pediatrician, and I specialize in treating acne. Thank you for registering to be a patient at Toronto Adolescent Acne Clinic.
Please fill out this questionnaire, so my clinic can create your medical record, and so I can assess your acne. It will take around 20 minutes to complete.
You will need the following to complete the questionnaire:
1. Your OHIP health card.
2. The name, address, and fax number of your preferred pharmacy.
3. One to four recent photographs of your acne.
4. The names of the prescription medicines you've used to treat your acne.
Once I have had a chance to review your responses, you will receive an e-mail that will let you book an appointment.
